I hereby agree to pay the registration fee for the {{formTitle}}. I understand that this fee is non-refundable. I understand that the {{formTitle}}s are operated and sanctioned by USSSA.
In consideration of my minor child being allowed to participate in any way for the {{formTitle}} and USSSA, its related events and activities, I acknowledge, appreciate and agree that: The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and, I knowingly and freely assume all such risks, both known and unknown, on behalf of my minor child even if arising from the negligence of the Releasees or others, and assume full responsibility for my child's participation.
Accordingly, I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided in this document and for myself, my child and our heirs, assigns, and next of kin, and hold harmless USSSA, its officers, directors, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to the person or property associated with my child’s presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I hereby authorize USSSA staff and representatives to act for me, according to their best judgment, in any medical emergency. As parent/guardian with legal responsibility, I take full responsibility for payment of injuries that may occur during this event.
I hereby grant permission to USSSA, its officers, directors, officials, agents and/or employees, and representatives to take and use photographs, digital images and/or digital media for broadcasting and live streaming of the minor child participating in this event for use in news releases and/or commercial or promotional materials including, but not limited to, websites, printed or electronic publications, other electronic communications and TV and/or internet broadcasts. I further agree that my child’s name and identity may be revealed in descriptive text or commentary in connection with the image(s). Since my child’s participation in this event is voluntary, neither I nor my child nor any member of my family will receive financial compensation for use of any images. I further agree that my child’s participation and the usage of any media image confers upon me and my child no rights of ownership whatsoever and relinquish any right that I or my minor child may have to examine or approve any media that these images may be used in. All negatives, prints, digital reproductions and other media shall be the property of USSSA. I release, discharge, and agree to hold harmless and defend USSSA, its contractors, clients, employees, directors, officers, legal heirs, agents, sponsors, advertisers, representatives and assigns and all persons acting under its permission or authority, from any liability by virtue of any reason in connection with the making and use of such photographs, including without limitation any claims for libel or violation of any right of publicity or privacy. I hereby warrant that I am a legal competent adult and a parent or legally appointed guardian of the minor, and that I have every right to contract for the minor in the above regard. I state further that I have read the above authorization, release, and agreement, prior to its execution, and that I am fully familiar with the contents of it. This release shall be binding upon the minor and me, and our respective heirs, legal representatives, and assigns.
Participants are required to furnish their own pants and other required field equipment including, but not limited to, gloves, bats, helmets, catcher’s gear and other protective gear.
